Practical Writing: Questionnaire

A questionnaire is a printed form containing a set of questions, especially one addressed to a statistically significant number of subjects as a way of gathering information for a survey. The following is a Questionnaire for Psychological Survey that has been filled in.

Instructions
Almost all of the questions asked in this survey use a seven-point scale as the method of response. This means you will see something that looks like this under every statement:

  Comments:

I like this kind of survey. But would you give me the result or the analysis as quickly as possible?
I also like the other kind of survey such as personality survey or self-perception survey.
